Please remember to submit your completed Medical Release Form to your team manager once your child is placed on a roster.

Also, if you are planning on doing any volunteering for the league, you must complete and submit a Volunteer Application Form (including your Social Security number).  The Volunteer Application Form should be sent to safetyfall@gmail.com.
